Handbook acknowledgement

By signing and submitting this form, both Athlete and Parent/Guardian acknowledge and certify that they:

Rower and Parent/guardian have read and understand the Boone Crew Handbook, the Boone Athletic Code of Conduct, and the BHSAA Alcohol/Drug Use Policy.

Rower and Parent/guardian have read and understand the academic performance policy as well as the consequences for not maintaining at least a 2.0 GPA.

Rower and Parent/guardian read, understand, and agree to the Positive Coaching Alliance’s Student-Athlete Social Media Agreement to use social media responsibly and respectfully.

Rower and Parent/guardian read, understand, and agree to the transportation and travel guidelines and agree to engage in team travel WITH the proper safety requirements in place, including valid drivers’ licenses, proper insurance, well-maintained vehicles, and compliance with all state laws.

Parent/guardian consent to have my rower transported by another rower and/or another rower’s parent/guardian to and from any BRI event as needed.

Parent/Guardian agree to complete Safe Sport Training and send certificate to registrar@boonerowing.com upon completion. (Training will be sent via email notification when it becomes available.)
